This form gives permission to the DSPS office to send your Reduced Course Load letter to the EOPS Coordinator.
The form is for students who are both EOPS and DSPS students.
The Reduced Course Load letter will be emailed to both the EOPS Coordinator and the student. Please submit a new request each semester.
