College2Career (C2C) supports approved education and employment goals of enrolled individuals with intellectual disabilities and Autism.
Eligibility
All students in the program must be a participant of the Department of Rehabilitation, be at least 18 years of age, and have a diagnosed intellectual disability or Autism. Program enrollment is limited each year. Admission is based on intake interviews and is made at the discretion of the C2C coordinator.
C2C students must:
- Be at least 18 years old
- Have a medical diagnosis for intellectual disability or Autism
- Be a participant (or eligible to be a participant) of the Department of Rehabilitation
- All students in the program must be a participant of the Department of Rehabilitation
Referrals
You can get a referral to join the C2C program from any of the following:
- DSPS counselor from Sacramento City College or any other college
- Alta California Regional Center coordinator
- Department of Rehabilitation counselor
- High School teachers or counselors
- Self-refer by contacting Tasneem Sah for an information meeting
- Open new cases each year, with new IPE’s through the Department of Rehabilitation
- Close cases as successful (closed-rehabilitated-successful employment)
- C2C will provide services to students, providing education planning and follow-through, vocational assessments as needed, employment preparation, job/internship development, and job retention services.
- College 2 Career will provide individualized case management for each participant to track progress throughout the semester.
- College 2 Career program students will have opportunities to develop job search skills, connect with employers, and enhance their career readiness through workshops, community partnerships workshops, recruitment events, tailored, individualized job search with C2C staff and volunteer work experience placements.
